Form & Identification:
Completed Registration Form along with a Copy of Birth Certificate / B-Form and Copy of Parent/Guardian CNIC.

Photographs & Health Records:
2 Recent Passport-size Photographs of the candidate and Vaccination or Immunization Card (Grade 3 till Grade 8).

Document Completeness Rule:
Incomplete applications will not be processed. Ensure all documents are complete and clearly readable before submission.
Payment Processing Note:
If your Online payment attempt fails (e.g. invalid OTP or card issue etc.), please do not resubmit the admission form. Instead, you may complete your pending payment directly here.

Online Registration Process

Manual Registration Process
OpenDownload the physical admission form using the link below. Submit along with fee (Rs. 3,000 cash) at Accounts Office from Jan 5th to Jan 16th.
* One submission process only is acceptable. Kindly submit your registration form via one method only (Either Online or Downloadable/Manual).
